Medication Form

All summer camp forms are completed during online registration except the medication form below.

This form must be completed, including a signature from your doctor, in order for your camper to self-administer medications.

Note: If your child is bringing ANY medication to camp, it must be in its original packaging and turned in to the camp health provider.
